We work with several major insurance providers to offer you comprehensive nutrition counseling benefits:
Most insurance plans do not require a referral for nutrition counseling. However, Medicare and Medicare Advantage plans do require a referral from your doctor.

Schedule your appointment online at your convenience. - Confirm and Prepare
After booking, we will confirm your appointment and send over the necessary paperwork. Please provide a copy of your insurance card at this time. - Insurance Benefits
We strive to confirm your insurance benefits, but each plan has different coverage limits. We encourage you to contact your insurance provider to verify that we are an in-network provider and to check the number of covered visits. - Prepare for Your Visit
To maximize the benefits of your appointment, upload your recent lab results to our secure portal or fax them to us at (908) 224-8994. This helps us tailor our recommendations to your specific health needs.

- Co-Pays, Late Cancellations, and No-Shows
Co-pays, late cancellations, and no-shows will be charged to the credit card on file. Please let us know as soon as possible if you need to reschedule or cancel your appointment.

Medicare clients do not need to verify their benefits as these benefits are standard for all Medicare beneficiaries.
Commercial: Each insurance plan works slightly different. We try our best to bill your insurance company under “preventative” benefits that typically do not require co-pays or deductibles. This is not always possible. The number of visits your plan allows also may vary. We review insurance cards prior to our first visit.
It is always best to call your insurance company to confirm your benefits. When verifying benefits.
Call the number on the back of your insurance card. Tell the representative that you would like to know your nutrition counseling benefits.
Make sure also to ask:
- Do I have a co-pay or co-insurance do at time of service?
- Is there a limit on the number of visits?
- Is telehealth covered?
- Is my dietitian in network?
The representative will ask you for the following information.
- Diagnosis code: Z71.3
- Procedure codes: 97802 and 97803
- NPI number of provider: 1144996398
If Happy Health Nutrition is considered “out of network” or we do not participate with your insurance, we can still work together. You will be charged at the time of service and we will provide you with a superbill (an receipt for your insurance) that you can submit to your insurance company for reimbursement of our services.
Each insurance company’s policy and way for submitting superbills is different. Call your insurance company to find out how to submit.
If you insurance does not cover our sessions we are still able to work together. Our initial 85 min session is 195 dollars and follow up sessions are 130 dollars.
We keep a credit card on file in case of co-pays, late cancellations/no shows and additional optional benefits like our concierge services (which is not covered by your insurance)
